Job Description:Job Summary:Primary responsibilities involve assembling, monitoring, maintaining, retrieving, and filing patient information in the electronic health record in accordance with established Health Information Management (HIM) policies and procedures. Release of information, HIPAA compliant. Electronic submission of birth certification to State of New Hampshire, Vital Records. Maintenance of Master Patient Index.JOB REQUIREMENTS:Education, Required Licenses/Certifications: High school graduate.Work Experience, Skills, Knowledge, and Abilities: Computer experience is required. Good command of grammar and spelling is necessary. Must possess strong verbal and written communications skills. Medical terminology is preferred. Prior experience in a hospital medical records department is preferred. Familiarity with computer applications required. Must have technical aptitude and a willingness to learn software applications used. Exceptional customer service focus.ESSENTIAL FUNCTIONS:
  • Tracks clinical documentation to monitor completeness and compliance with policies, as well as CMS requirements
  • Oversees use of electronic medical records and provides IT support to clinical team as needed
  • Compiles and submits reports for compliance audits
  • Assembles admission packets for all programs and compiles clinical record for new admissions
  • Tracks physician orders to monitor compliance with policies. Generates “late” physician order reports
  • Assists with reception and incoming phone calls
  • Accurately files clinical documentation within time frames
  • Assist Intake department with data entry
  • Disassembles discharged clinical records and tracks missing documentation
  • Assists with monthly pre-billing verification
  • Process Insurance and outside requests for medical records
  • Oversee HIPPA compliance for agency
  • Demonstrates good organizational skills
  • Uses equipment and supplies in an appropriate and cost-effective manner
  • Submits accurate time records according to policy
  • Acknowledges that all information related to employees, clients and their families, and business practices is confidential
  • Practices proper body mechanics and observes safety measures
  • Adheres to all safety policies including reporting/removing hazards immediately
  • Demonstrates knowledge and compliance with infection control policies
  • Follows instructions, responds to management direction
  • Works cooperatively with other personnel
  • Complies with Code of Conduct
  • Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations.
