- Directs patient care by assessing, planning, executing, administering, and evaluating treatments and procedures for patients. Coordinates acquisition of outpatient records from primary care provider or referring provider.
- Consults, educates, and monitors patient evaluation and treatment decisions. Acts as a liaison and facilitator between hospitalist, staff members, case managers, and attending physicians regarding patients.
- Rounds with hospitalist ensuring all lab work, diagnostic results and other test results are available on chart and notifies hospitalist as appropriate.
- May be involved in auditing or evaluating process improvements in the quality of patient care and nursing research or assist in designing tools and systems to optimize nursing patient care.
- Acts as an internal and external consultant to promote clinical and patient care standards. Facilitates coordination and development of a comprehensive and individual plan of care in collaboration with patient, family and multidisciplinary team.
- Manages/produces/coordinates "MD TRU" rounds every morning. Checks admission criteria and prospective plan for obs. Acts as a hospitalist service liaison with patient care staff.
- Facilitates one on one communication with primary care physician regarding current and ongoing patient care needs. Follows up discharged patients within 48 hours of discharge. Copies and sends appropriate discharge paperwork to primary care physician. Works with case managers to track avoidable days and adverse events.
- Participates in development and surveying for staff satisfaction. Verifies PCP/referring physician notification of admission. Works with clinical pharmacists to ensure most up to date medications from home.
- Navigates patients through the healthcare system based on individual and patient population needs. Directs liaison activities to integrate patient in the healthcare continuum including procuring of services, health promotion and counseling, and prevention screening. Applies approved criteria to monitor admissions and continued stay. Monitors implementation of guidelines for resources, trends and tracks variances. Coordinates the use of outside agencies and community resources in establishing maintenance plans.
- Track hospitalist program data on a monthly basis. Acts as a liaison of clinical staff and physicians and staff on integral criteria. Analyzes and synthesizes all data related to catastrophic cases including data related to quality of care as well as cost effectiveness.
