The Population Health Navigator will collaborate with the interdisciplinary team to grow, develop, and support the preventative health measures that support compliance by our patient population. Duties will include triaging and facilitating care to patients, patient and staff education, data collection, protocol development, ensuring compliance with guidelines and current measures by insurance payor. The Population Health Navigator must be able to work in a self-directed environment, with the willingness and ability to collaborate with other team members for primary care and the specialty clinic.JOB DUTIES:· Is accountable for the quality measures as the primary contact for all insurance payors and to CRMC Administration.
· Functions independently providing direct and indirect patient care based on the clinic processes for the patients in CRMC Medical Group.
· Will collaborate with all providers involved in the patient care treatment plan by utilizing knowledge of patient assessment, planning, intervention, evaluation, and insurance payor databases.
· Assist in having patient complete diagnostic tests ordered or prescribed by provider in timely manner.
· Assess and evaluate patients' needs for, and responses to, care rendered.
· Collaborate with Chronic Care manager to provide education as needed to the patients/family related to their chronic care diagnosis, diagnostic procedures, medication, and other complex aspects of the plan of care when appropriate.
· Collaborate with the clinical team to create a Plan of Care for all patients.
· Maintain confidentiality of all information according to HIPAA (Health Insurance and Portability and Accountability Act) and company policy.The Population Health Navigator assumes other responsibilities as assigned.JOB REQUIREMENTS:

  • Accurate clinical assessment skills regarding patient medical conditions.
  • Ability to effectively communicate with patients, families, physicians, and hospital staff.
  • Computer skills including MSOffice, Outlook, Meditech, preferred.
  • Ability to read, analyze, and interpret common scientific and technical journals.
EDUCATION AND EXPERIENCE:
  • CMA or clinical experience preferred.
  • Valid driver’s license, required.
  • Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties.
  • Familiar with database metrics for quality reporting and tracking
  • Associates degree in healthcare field, preferred
