Position Description
Overview
The Care Coordination Supervisor, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE’s requirements. At CINQCARE, we do not have patients or customers – we have Family Members.Duties and Responsibilities of the role * The Care Coordination Supervisor (CCS) manages a team of Care Coordinators who provide comprehensive assessment and evaluation, care planning and coordination, outreach and crisis intervention, linkage to medical, financial, social, rehabilitation and psychiatric services, advocacy, development of social supports and procurement of needed resources.
  • The CCS monitors the performance of the Care Coordinators and assures that the team/program outcomes are met, including the completion of required documents by the designated deadlines.
  • The CCS is responsible for overseeing the engagement activities, comprehensive assessment and service planning the team provides to individuals who have chronic medical problems or significant mental illness/substance abuse.
  • The CCS provides, at minimum, biweekly supervision of each individual team member and facilitates weekly virtual huddles and once a month in person team meetings. Weekly supervision will be provided to a Care Coordinator if deemed necessary due to performance.
  • The CCS will accompany team members on planned and unplanned home visits as needed or required.
  • The CCS will provide field supervision as needed and complete field evaluations.
  • The CCS will ensure timely completion and delivery of annual performance evaluations of team members.
  • The CCS reviews concerns regarding team members with the Director of Care Coordination and submits disciplinary action and/or corrective action plans for team members where appropriate.
  • The CSS will participate in management and systems training and development as required.
  • The CCS assures that Agency/Program safety protocols are adhered to by team members.
  • The CCS will participate in the bi-weekly management meetings, supervision meetings, and other agency meetings as assigned.
  • The CCS will audit charts as needed or requested by the Director of Care Coordination and/or the Quality and Compliance Team.
  • The CCS ensures that team members respond promptly to all communication related to consumers, internal correspondence, or outside communications.
  • The CCS will manage all complaints from family members, document them appropriately and follow up as needed.
  • The CCS will stay up to date on Health Home policy changes and the standards and workflows related to those policies.
  • The CCS will contribute to the team culture and moral through leading by example, facilitating and participating in team building activities and addressing toxic behaviors.
  • The CCS will foster a culture of inclusivity through facilitating and participating in DEI discussions and activities.
  • Perform other job-related duties as assigned.
Qualifications
The Care Coordination Supervisor should have the following qualifications:
Education:
  • Master of Social Work, Mental Health Counseling, Psychology, or other Human Services related field is preferred or a RN.
  • Bachelor’s degree in a human services related field is required.
Experience:
  • Minimum of 3-5 years of providing direct care coordination services to individuals living with Serious Mental Illness (SMI) and/or substance use disorders (SUD) and/or people living with HIV and/or people with chronic medical conditions.
  • Minimum of two (2) years’ experience in a management position providing supervision and oversight of team members
Required:
  • Valid and unrestricted NYS Driver’s License.
  • Insured vehicle in NYS

  • Knowledge: A good understanding of healthcare systems, services, and resources is essential. Knowledge of Health Home regulations and policies, as well as relevant laws and regulations, is essential. Strong data management and analysis skills is beneficial.

This includes excellent verbal, written communication, and presentation skills. They should be able to listen actively, communicate clearly, and collaborate with multidisciplinary teams.
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
  • Cultural Sensitivity: Care Coordination Supervisors must have a cultural sensitivity and be able to work with diverse populations. This includes understanding and respecting individuals' cultural, ethnic, and socioeconomic backgrounds to provide appropriate care and support.

  • Critical Thinking and Problem-Solving: Care Coordination Supervisors must have the ability to assess complex situations, identify needs, and develop appropriate action plans. Strong critical thinking and problem-solving skills are essential to address team member challenges.
  • Organizational and Time Management Skills: Care Coordination Supervisors manage multiple team members and tasks simultaneously. Strong organizational skills and the ability to prioritize, manage time effectively, and meet deadlines, are crucial to succeed in this role.
