Admission/Discharge Nurse LPN/RN

- Complete 2/3 admissions per day
- Complete Admission Check List within 24 hours (some items must be completed same day)
- Assist with completing consents
- Coordinating discharge
- Referrals to outside resources
- Ordering/ensuring equipment in place
- Discharge medication reconciliation
- Ensuring teaching in place-completed/starting on admission
- Review for medical stability and coordinate SBN need with MDS coordinator
- Complete discharge planning care plan specific to patient needs
- Discuss AMA with physician and ensure AMA form signed
- Complete discharge paperwork
- Complete Recap day of discharge
- Complete D/C GG on Medicare A
- Opens discharge med list on J drive 48-72 hours prior to discharge
- Schedule and attend discharge care conferences
- Attend Medicare meetings
- Assist with MDS as needed/cover for vacations
- Must have the ability to cooperate with others and use tact and patience in dealing with residents, their families and visitors.
- Ability to be flexible and be capable of adapting to frequent changes in routine.
- Experience as a general staff nurse, in which administrative and supervisory ability has been demonstrated.
- Graduation from an accredited school of nursing and current registration/ licensing with the State Board of Nursing Examiners
