Additional benefits include Employee Assistance Program and Education Reimbursement program.Full-time and Part-Time (over 20 hours/week) accrue PTO at rate of 0 .034/ hour during their first year.POSITION SUMMARYAn Assisted Living Licensed Practical Nurse (LPN) provides care to residents in an Assisted Living or Memory Care community, under the supervision the Health Services Director (HSD) or a Registered Nurse, and as prescribed by the physician, by following the care plan, administering medications and treatments. Promotes a positive physical and psychosocial environment for the residents in compliance with all local, state, and federal regulations.ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Performs nursing treatments and administers prescribed and PRN medication(s) in accordance with the resident care plan and as prescribed by the physician and with approved nursing techniques. Reports resident response to nursing interventions and informs Health Services Director (HSD) of observations.
  • Responsible for the daily operation of the medication room and medication services as directed by the HSD and in accordance with physician orders, state regulations and the nurse practice act.
  • Evaluates residents for changes of condition and reaction to medications and treatments, documents, and notifies HSD and physician of changes and significant incidents.
  • Takes temperature, pulse, blood pressure, and other vital signs and observes skin, cognitive functioning, and behaviors to detect deviations from baseline and report on the condition of residents.
  • Reports all medication errors per policy. Completes notifications timely and all required forms.
  • Documents all care provided and observations in a timely and accurate manner including charting vital signs, medications and treatments given, writing progress notes, and completing incident reports.
  • Transcribes and enters orders into the resident's records and seeks clarification of unclear orders from the ordering physician and/or other health care professionals. Coordinates with pharmacies for medication orders and refills.
  • Responds to resident call lights, and assists with transfers, ADLs, and other personal care tasks as needed.
  • Assists with assessments or evaluations of health, functional and psychosocial status of residents. Participates in the development of individualized service plans. Problem-solves as part of the management team to case manage and meet the needs of residents.
  • Communicates regularly with residents' families regarding health care needs. Provides education and support to residents and families.
  • Partners with the Health Services Director to ensure that resident care needs are met by coordinating services with outside providers and professionals.
  • Serves meals in the dining room, assisting residents at mealtime, provides resident tray delivery, and assists with dining room and kitchen clean up and maintenance.
  • Assists in lifting, moving, and transporting residents using proper body mechanics or assistive devices as directed by the resident's cooperative service plan.
  • Helps with tours, move-ins, move-outs, and transfers.
  • Assists in the management of infection control. Provides training to staff on infection control practices, investigates, and reports incidents of infection and tracks infection data.
  • Provides on-call support and phone triage for team members' questions regarding resident care. Provides guidance based on resident care plan and Prestige policy and procedure.
  • Assists with staffing and scheduling of health services staff as directed.
  • Maintains up-to-date knowledge of all rules, regulations, and policies pertaining to resident care. Responsible to stay informed of nursing practices and standards and apply new knowledge to meet the changing needs of residents.

QUALIFICATIONS AND EXPERIENCE
  • Must have a current, unencumbered Licensed Practical Nurse license in state of practice, or the ability to obtain one before practicing in the state.
  • Experience working in long term care including medication administration preferred.
  • Ability to creatively solve problem solve resident and staff member situations.
  • Must be a team player, well organized, and flexible.
EDUCATION
  • Licensed Practical Nurse certification.
