Our commitment to excellence extends to every aspect of our operation, including the coordination and implementation of the Resident Assessment Instrument Process (MDS). We are currently seeking a skilled and experienced Registered Nurse to join our team as a Skilled Nurse Assessment Coordinator.Location: Sartell, MinnesotaPosition Overview: The Skilled Nurse Assessment Coordinator (SNAC) is responsible for the coordination and implementation of the Resident Assessment Instrument Process (MDS) in accordance with state and federal regulations. The SNAC will communicate and coordinate health information to skilled payers, ensuring accurate and optimal reimbursement for resident care. The ideal candidate will have current RN licensure in the State of Minnesota, with two or more years of RN experience, along with a strong understanding of the MN Case Mix System, MDS process, and general resident assessment requirements. Country Manor is willing to provide training for qualified candidates.Key Responsibilities: * Resident Assessment Coordination: Coordinate and oversee the completion of the MDS assessment process for all residents, ensuring accuracy and compliance with regulatory requirements.
  • Communication with Skilled Payers: Communicate assessment findings, care plans, and resident progress to skilled payers, ensuring accurate reimbursement and continuity of care.
Qualifications:
  • Current RN licensure in the State of Minnesota.
  • Two or more years of RN experience, preferably in a skilled nursing or long-term care setting.
  • Knowledge and understanding of the MN Case Mix System, MDS process, and general resident assessment requirements.
  • Ability to communicate effectively with skilled payers, and other members of the healthcare team.
  • Knowledge of Medicare, Medicare Replacement, and Medicare Covered reimbursement systems preferred.
