Sun River Health is currently seeking a full-time Genesis Care Manager to support our HUB site in The Bronx.$1500 Sign On Bonus- Terms and Conditions Apply****SUMMARY OF POSITION:The Genesis Care Manager will work as part of a specialized team within the Primary Care setting of Sun River Health to provide medical case management services to persons living with HIV and/or Hep C. Services include but are not limited to: assessing patient needs; service plan development; care coordination; referrals and linkages; and education and support. Staff will utilize motivational interviewing and a harm reduction approach to provide trauma informed care based supportive interventions.ESSENTIAL FUNCTIONS:· Completes patient assessments and reassessments utilizing a strength based model· Provides health education, risk reduction and adherence education to patients within the scope of discipline to assist patient in engagement and retention and reduce barriers to care to enhance health outcomes· Conducts outreach to re-engage patients in care· Participates in ongoing training and regional collaboratives to enhance knowledge and skills· Participates in quality improvement trainings and activities to improve patient care· Demonstrates cultural competency in working with diverse populations using an anti-racist framework and equity lens· Performs job duties in compliance with systems and grant specific standards· Maintains data and case records as required· Develops, implements and updates care plans for Genesis patients· Facilitates enrollment in benefit and entitlement programs· Participates in case conferences presenting patient needs within a strength based model· Develops linkages and refers patients for additional supportive services· Conducts intakes and verifies and documents program eligibilityThe above is intended to describe the essential job functions, the general supplemental functions and the essential requirements for the performance of the job it is not to be construed as an exhaustive statement of all the job functionsEDUCATION/EXPERIENCE:Bachelor's Degree required.2 years of related work experience.Job Type: Full-timePay: $23.00 - $28.75 per hourResponsibilities:Relation to Mission
