- Act as primary resource and offer support to participants and families through the OnTrackNY evidence based model and differentiate care through phases of treatment: initial engagement, ongoing treatment, and Graduation (discharge) from OnTrack
- Using active outreach and engagement to build relationships and maintain retention with participants and families
- Use OnTrack evidence based practices, such as Core Sessions and Recovery Coaching, as well as therapeutic approaches based on Strengths Based Case Management, Motivational Interviewing, and Cognitive Behavioral Therapy
- Assess for safety and risk using evidence based models and support in safety planning and relapse prevention planning
- Support participants holistically. Including but not limited to: medication use/management, medical care, health and wellness, education/employment, family housing/income, trauma-informed care, and suicide prevention
- Collaborate with participants and team members on assessments, treatment planning, safety-planning, wellness self-management, and transition planning
- Communicate and collaborate with the team actively and regularly via e-mail, telephone, and team meetings
- Rotate on-call after-hours access responsibilities for crisis intervention purposes with other team members
- Master’s degree in Social Work (MSW) or Licensed Master of Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Trained and supervised in evidence-based practices
- 2-3 years of relevant experience with similar populations, including working with young adults, supporting individuals with experience of altered perceptions
- Supporting individual’s families and social supports; utilizing a strengths-based, culturally affirming approach
- Bilingual candidate preferred but not required
- Knowledge of NYC and New York State community organizations and case coordination
- Travel to participants or off site locations
