We have an exciting opportunity to join our team as a Supervisor, Social Work.In this role, the successful candidate Provides psychosocial assessments and treatment as well as consultation about patients and families to assist them and
the Health care team in coping with the patient's hospitalization, illness, diagnosis, treatment, and/or life situation,
including emotional, mental, and substance abuse disorders.Job Responsibilities:Provides psychosocial assessments of patients and families to identify emotional, social, and environmental
strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Formulates, develops,
and implements a plan utilizing appropriate social work interventions. Documents findings, plan, and actions taken
according to departmental and institutional guidelines and standards.
- Performs other duties as assigned
- Attends and participates in staff, committee, departmental, institutional, and other administrative meetings and
- Maintains records and statistics according to departmental and institutional guidelines and standards.
- Participates in multi-disciplinary health care teams and provides leadership in representing the social work
- Provides education to patients and families around issues related to adaptation to the patient's diagnosis, illness,
therapies. Provides teaching and training for students and staff from social work and other disciplines.
- Arranges, procures, and coordinates patient and/or family pre and post hospital or treatment needs.
- Treats and manages mental health and/or substance abuse problems. Assists with screening, identification,
consultative services to other health care professionals.Minimum Qualifications:
To qualify you must have a Master's Degree Social Work. Current NYS Clinical Social Worker License. 3 - 5 years Post Master's Degree experience
in Health or social agency. Excellent communication (both oral and written), interpersonal, organization, leadership, and
problem solving skills. Ability to provide assistance based on physical, psychosocial, educational, safety and related
criteria, appropriate to the age of the patients served. Thorough understanding of social service operations within the
