We are seeking LPN's to work with our developmentally disabled and brain injured clients in their residential programs. The Nursing Team advocates and institutes the appropriate nursing interventions, for all medical needs of assigned program participants focusing on optimal health for all individuals..Position Responsibilities:
  • 1:4 nurse/client ratio.
  • Monitor and coordinate healthcare services for participants. Institute appropriate follow up and communicate all medical issues to: Physicians, nurse practitioners, and other medical licensed providers, Area Director, Nursing RN Supervisor/ and or Health Care Director and any other appropriate DDS staff.
  • Document all nursing interventions and changes affecting the individual in appropriate areas - Daily log, progress notes, staff communication log, Nurses Notes, and any other identified system for communication purposes.
  • Maintain current medication and treatment orders according to MAP standards noting any side effects/adverse effects and report to appropriate people.
  • Assist in training staff in personal care, positioning, feeding, use of supportive devices.
  • To assist the individuals with their personal care and ADL's, and community integration being mindful for allowing ongoing opportunity for individuals to assume increasing control and responsibility.
  • Accompanies patients to medical appointments/Day Programs.
  • Additional assignments required / directed by management.
.Qualifications:
  • High School Diploma / GED required.
  • College Degree / Certificate preferred.
  • Valid MA Nursing License.
  • Valid driver's license and acceptable driving record.
  • Physical ability to perform the requirements of the position.
  • Good communication and computer skill.

I grew up on a farm, showed dairy cows when I was in high school, studied Animal Science at Smith Voc, and worked on several farms raising llamas, goats and alpacas before I came to Prospect Meadow. And I've since learned at least as much from the farm hands as I have brought to them. They are an incredible group of caring, curious, nurturing people who always want to know how I am, how my dog is; and they are eager to learn what I share with them about farming. They've taught me how to communicate very specifically, to include important details, and to check to make sure I've been understood-not just to assume. And they've taught me how to slow down, take a breath, think.

These are the same approaches I've shared with parents here at REACH and in Nigeria when I went back to work with the government there on developing Early Intervention services. Parents are parents anywhere you go. My job is to help them help their child, advocate for their child, and move forward on this journey.
