Let your career find its purpose here at Carondelet.Clinical Liaison Referral Coordinator FT Days Position SummaryPoint of contact position for responding and acting upon referrals from Carondelet hospitals and the community. Liaison with case managers, physicians and community seeking acute rehabilitation services for patient/family member. Obtains necessary medical information to present to Medical Director to assist with admission into acute rehabilitation program. Coordinates admission details with nursing manager, therapy supervisor and patient's family. Coordinates insurance approval for the admission. This position is accountable to the program director of the inpatient rehabilitation program.ResponsibilitiesUses basic knowledge of Rehabilitation Prospective Payment System (PPS) terminology and applies this during the prescreening process.
Navigates Microsoft Office, Meditech & Cerner software efficiently.
Understands rehabilitation admission criteria and applies it appropriately during assessments of patients referred for inpatient rehabilitation services, including making recommendations for alternate levels of care within the continuum to referral sources if the patient is not a candidate for admission.
Initiates meetings with patients and /or families, physicians, and all other referral sources to inform and educate regarding the rehabilitation process.
Ensures accurate completion of pre-admission screening forms.
Ensures accurate documentation of clinical findings in the medical record.
Participates in clinical rounding where appropriate.
Provides support for the rehabilitation Medical Director by gathering data for internal and external inpatient rehabilitation referrals from various sources including the medical record, onsite and telephonic interviews, and through other electronic media.
Coordinates information with the Medical Director, patient admissions coordinator, nursing unit manager(s), and other team members in a timely and effective manner to advocate for patients by providing the fastest decision regarding admission.
Collaborates with team members in the admission decision making process quickly and effectively to discern time of admission, patient bed placement, and any special needs of the patient to provide a tmey turnaround for referral sources.
Facilitates the admission process with the payer sources and the admissions coordinator for inpatient rehabilitation.
Manages all referrals, admissions, and denials from a proactive approach and exhibits behavioral skills that promote a positive customer relations environment.Level 2 - Proficient (in addition to above duties)
Demonstrates knowledge of current clinical care requirements and directives including regulatory standards.
Knows ICD-10 coding and how the codes affect the tier assignment for each case.
Understands and contributes to targets/benchmarks for success of the rehabilitation program.
Coordinates and conducts educational presentation(s), internally and externally.
Develops and maintains physician relationships both internally and externally.Level 3 - Subject Matter Expert (in addition to above duties)
Applies proper application of the rules and regulations for IRF's to the facility / program.
Participates in identifying trends and issues within the facility to promote improved clinical outcomes.
Participates in community health organizations related to rehabilitation services.THE CLINICAL LIAISON REFERRAL COORDINATOR FT DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.LICENSURE/CERTIFICATION/REGISTRATION
License Required: Valid and current Arizona nursing license, or compact state license and ability to become Arizona Licensed in 30 days; or related rehabilitation field license.
Certification Required: CPR-BLS
Certification Preferred: Functional Independence Measures (FIMS).EDUCATION
Required: Associate's Degree in Nursing or Healthcare related filed.
Preferred: Bachelor's degree (BS, BA) in healthcare related field.WORK EXPERIENCE
Required: Two (2) years recent acute care, community, or long term care rehabilitation clinical experience.#LI-SW2Responsibilities:Process patient referrals for the practices in the market, Facilitates orders outside to outside facilities. Coordinate scheduling of office visits for the referred patient Assist patient with finding resources that accept the various insurances Obtains pre-authorization from payers. Calls insurance companies to get medications approved that may have been denied due to formulary changes.Qualifications: Education:Required: High school diploma/GED.
Preferred: Completion of a medical assisting or medical office assistant program. Experience:Required: Must have a minimum of 4 years of experience working in a medical office setting.
