TITLE: Optometrist
REPORTS TO: Director of Optometry with oversight by Chief Medical Officer
WORK WEEK: Scheduled in the clinic 4 10-hour days per week, including Saturday.
WAGE CLASSIFICATION: Exempt, salaried.
OSHA RISK CLASSIFICATION: HighMISSION: Strengthening our community by providing accessible and affordable healthcare.BENEFITS: AK licensure reimbursement, medical & dental insurance, PTO, floating holiday, CE time off and funds, life, AD&D and long/short term disability, 403b, access to NHSC and State of AK SHARP loan repayment program.SUMMARY POSITION STATEMENT This position exists to provide comprehensive vision care to the patient population in the Central Kenai Peninsula so they can care for their eyes and access tools to support their eyesight.ESSENTIAL FUNCTIONS/ROLES & RESPONSIBILITIES OF THE POSITION
  • Evaluate and treat patients within the privilege range granted by the PCHS Board, and within the scope of training, capabilities, and license criteria as an optometrist
  • Conduct routine eye exams for patients
  • Prescribe medications to treat ocular diseases and disorders
  • Perform pre-and post-operation checks
  • Check patients for signs of diseases and refer them to other specialists when needed.
  • Be a patient advocate.
  • Coordinate health promotional activities.
  • Provide patient follow-up as necessary for continuity of care.
  • Coordinate care with other health care providers when referrals are necessary.
  • Be familiar with and follow adopted PCHS community health center standards of care.
  • Maintain accurate, complete, and timely documentation regarding patient contacts within (EMR) patient charts. Charting to be done during a normal workday.
  • Assess grant criteria and incorporate into the patient encounter.
TEAM DUTIES & RESPONSIBILITIES OF THE POSITION
  • Analyze population data to identify high leverage interventions with their patient panel.
  • Assist in facilitating coordination of care
  • Develop and implement a plan of care and recommend interventions to attain optimal patient function
  • Effectively utilize electronic health record (EHR) to document clinical activity and manage patient population
  • Work effectively and cooperatively with other members of Care Team
  • Participates in Care Team training and learning activities
  • Assist in facilitating coordination of care.
