The Case Manager is responsible for the case management of clients enrolled in the Family Assistance Program. Communication required with key staff such as Family Assistance Program Coordinator, Lead Case Manager, Case Managers, Counselors, and Catholic Charities staff. Collaboration with agencies such as the City, State and Federal agencies, Donors, community members, volunteers and Catholic Charities staff is required.The Expertise We Are Looking ForThe Expertise We're Looking ForAssociate Degree; orTwo (2) years of experience in lieu of educationValid Texas Driver’s LicenseThe Skills and Assets You Bring, and MoreYou have knowledge of Social Work Principles and EthicsYou have clinical knowledge in vulnerable populations and guiding them into self-sufficiency.You have experience working with individuals and families in crisis and trauma.You have the ability to work collaboratively with all stakeholders and maintain professional courteous demeanor.You have the ability to keep accurate client files, handle and maintain confidential information.You have competence in ETO Data Quality and Reporting an asset.You have working knowledge of case management systems and planning techniques.You have working knowledge of Word suit - word, excel, power point.What you DeliverYou manage the delivery of case management services to Family Assistance Program clients in Harris, Fort Bend or Galveston counties. Provide Lead Case Manager or Family Assistance Coordinator with innovative concepts to maximize the impact of case management service delivery and a plan of integration with the overall vision of the Family Assistance Program and Catholic Charities. Continually monitor and evaluate each client's progression through their case management plan, developing corrective action revisions to the plan as needed; Participate in all client treatment team meetings where appropriate.You maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Catholic Charities guidelines; Review/ Audit of client files (hard files and ETO) to ensure case management plans address identified needs, are up-to-date and that data quality is maintained; Manages compliance related monitoring and audits ensuring that compliance goals are met and/or exceed requirements and makes service delivery adjustments as needed. Record reviews are completed quarterly with each program maintaining a level of at least 85% for each standard and that action plans are developed to ensure standards are met. Prepares monthly PIR and Bridges to Hope dashboards.You complete case management components of reports, required by the funders or other stakeholders with required content and within the established deadlines. Ensure that all programs comply with appropriate standards, (CCGH, CCUSA, COA, UW, and FAP). Plan and participate in program and agency related meetings, team meetings and external meetings as required. Complete required hours and topics of annual training and assess for/recommend additional trainings to assist in professional development.You perform other duties as assigned by the Family Assistance Program Coordinator or Lead Case Manager, such as participating in staff retreats, program tours, agency fairs and speaking engagements, assisting with special organizational projects/events, etc.Catholic Charities is an equal opportunity employer.
