You will be responsible for delivering psycho-social care to patients whose physical, emotional, psychological, social and economic functioning have been affected by the onset of illness. Patients and their families are helped to cope with illness through interventions such as psychotherapy, supportive counselling, grief counselling, financial assistance, restoring and expanding patient's social support networks, and linking patients and their families to relevant community resources and support. In addition, this role would require the applicant to:

Responsible for making social visits
Conduct interviews with patients and family members
Assist in identifying and resolving issues
Liaise with government departments and social service organizations

Requirements

Degree in Social Work
Min. 3 to 5 years of relevant experience
