Meath Day Service

Permanent Part-Time Contract, (35 hours)Job Purpose
Links - is a new service commissioned by the HSE South and aimed at providing a range of supports to School Leavers and RT leavers. The purpose of the job is to promote and support independent living and community integration personal development and work opportunities for service users. To assist the Community Services Manager in the development, implementation and delivery of quality customer focused and cost effective community ser-vices to service-Users at an appropriate time and setting. This includes the provision of individual programmes and activities to meet the needs, choices and abilities of service-users, and an active contribution towards the involvement of RehabCare in the expansion of this service.Minimum Education & Skills required
- A Third level graduate, health and social care equivalent
- MAPA, Autism Awareness Training, Occupational First Aid, and Manual Handling de-sired.
- Experience of working with adults with disabilities
- Experience of the design and delivery of individual programmes for people with disabilities or those who are marginalized
- Well-developed people management skills, with proven experience of building, leading and motivating a team.
- Experience in the promotion and support of independent living and community integration for service users.
- Build trusting relationships and communication with service users, facilitating them to identify their goals, strengths and needs.
- Support service users to establish and enhance their natural support circle.
- Supporting service users to identify the community based supports they would like to avail of and work with service users to identify practical solutions to any barriers to ac-cessing these supports.
- Provide service users with accessible information on available community based sup-ports, ensuring all activities and supports are monitor and are maintained at an appro-priate pace to ensure sustainability.
- A flexible shift system will operate
- A full, clean driving license, with a minimum of 2 years' experience is required.
