- Perform risk assessments and conduct initial and comprehensive evaluations. Develop and update action plans and produce session and end-of-service reports.
- Conduct both one-on-one sessions and group work or program facilitation as outlined in action plans. Maintain accurate records in compliance with the Refer and Monitor system requirements.
- Establish and nurture productive relationships through effective communication and liaison with all stakeholders. Offer guidance to relevant agencies to improve access and support for the POP program.
- Collaborate closely with probation and substance misuse services.
- Provide assistance with attending community appointments.
- Participate and contribute to key meetings such as team meetings and multi-disciplinary reviews, sharing information appropriately.
- Proficient understanding and background in the criminal justice sector.
- Extensive expertise in the substance misuse field or related areas.
- Demonstrated ability to handle substantial caseloads, conduct risk/comprehensive assessments, develop SMART action plans, and maintain effective case records.
- Proven track record in delivering both one-on-one and group interventions utilizing motivational techniques and trauma-informed approaches.
- Previous involvement with treatment and/or healthcare services.
- Capability to establish and maintain professional boundaries.
- Effective collaboration with key partners to achieve successful outcomes.
