- Provide Registered Nurse services in support of the Medical Home Port (MHP) Primary Care Clinics at the Naval Hospital Jacksonville, FL. The objective of this program is to enhance continuity of care, wellness, and disease management for patients.
- The health care worker (HCW) will be assigned to a primary location; however, the Government retains the right to assign to another location within a fifty (50) mile commuting radius of their assigned Military Treatment Facility (MTF). Shall be notified two weeks in advance prior to reassignment to locations within a fifty-mile limit.
- Degree: Possess Associates Degree of Nursing
- Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).
- Experience: One year of experience in nursing after graduation
- Certifications:
- Possess and maintain current Basic Life Support (BLS)
- Advanced Cardiac Life Support (ACLS)
- Pediatric Advanced Life Support (PALS) Certifications
- Licensure: Possess and maintain a current unrestricted license to practice as a registered nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
- CME Requirement: Have successfully completed at least twelve (12) classroom hours of continuing education within the preceding twenty four (24) months that maintains skills and knowledge as an RN.
- U.S. Citizenship: HCWs performing under this contract shall be U.S. citizens.
- Able to read, write, and speak English well enough to effectively communicate.
- Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required.
- Provide three (3) letters of recommendation from practicing physicians or peers attesting to his/her clinical skills.
- Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference.
- Letters of reference must have been written within the preceding two (2) years.
- Coordinate patient care in collaboration with a wide array of healthcare professionals.
- Facilitate the achievement of optimal outcomes in relation to clinical care, quality and

- Administer medications, wound care, and numerous other personalized interventions.
- Direct and supervise care provide by other healthcare professionals.
- Accountable for making patient care assignments based on the scope of practice and skill
- Communicate and collaborate with a diverse group of people for the purpose of
patient/family and organization. * Make referral appointments and arrange specialty care as appropriate.
- Conduct research in support of improved practice and patient outcomes
- The Primary Care Clinics are open from 0630 until 1900, Monday through Friday.
- Shift lengths may be variable, 8.5 or 9-hour period (to include an uncompensated 30 to 60 minute meal break, respectively) to ensure shift coverage.
