- Coordinating and overseeing the mobilisation of new complex care packages
- Managing the process of new package set-up including care plans, risk assessments, and compliance with regulatory requirements
- Developing relationships across the business to ensure the bespoke service runs seamlessly
- Liaising with key stakeholders including clients, families, healthcare professionals, and internal teams to ensure a smooth and timely delivery of care
As a Care Coordinator, you will receive:
