Primary Purpose:The person in this position is a member of the Patient Centered Medical Home where each team member plays an integral role in providing patient-centered health care. The incumbent supports the care team in completing and coordinating various clerical tasks to ensure smooth and efficient patient care.Essential Duties:· Schedules transportation services for patients, ensuring timely and efficient transportation to and from healthcare facilities.
  • Receives incoming phone calls, provides basic information in accordance with the direction of the care team, and assist with resolving inquiries or redirecting calls to the appropriate healthcare professionals.
  • Following up on Durable Medical Equipment (DME) referrals, and requesting records from external providers.
  • Collaborates with the pre-visit reviewer to ensure that all documents are accessible in EMR prior to visit, including but not limited to timely retrieval of consultation and transitional care reports
  • Works with the assigned medical assistant in following up patients who chronically no-show to their appointments
  • Schedules appointments, coordinates, and follow up internal and external patient referrals as necessary.
  • Reminds patients about upcoming appointments through phone calls or other communication channels, ensuring they are aware of the date, time, and location of their appointments.
  • Schedules and coordinates ER/Hospital follow up appointments as necessary
  • Refers patient to RN care coordinators for further education or follow-up as needed
  • Works together with the team in identifying and closing gaps in patient care
Other Duties:· Performs other related duties as assigned· Observes safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.Minimum Qualification Requirements:Skills/Knowledge:· Strong communication skills, both verbal and written, with the ability to effectively interact with patients, healthcare professionals, and external service providers.· Flexibility to adapt to changing priorities and work effectively in a fast-paced environment.· Ability to maintain confidentiality and handle sensitive patient information with professionalism and discretion.Education:· High School Diploma/GED equivalent required· One year certificate from college or technical school or equivalent combination of education and experience preferred· Diploma or certificate as a Nurse aide, Medical Assistant, Emergency Medical technician, or Patient Care Technician preferred· RN with a foreign RN license or Nursing student preferredExperience:· New graduate welcome or a general experience in working in a community or hospital setting preferred· Previous experience in a clerical or administrative role, preferably in a healthcare setting.Kalihi-Palama Health Center is an equal opportunity employer that is committed to diversity, and values the ways in which we are different.
