Job SummaryWe are looking for a Home Health and Hospice CNA to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.Shift is Monday-Friday, 4pm-7pm in Bountiful for 3 Special Need's patients.Responsibilities
  • Perform client personal care needs and support other clients and family every-day-needs as supported by the hospice plan of care
  • Record and report important health information including temperature, heart rate, respiration rate and/or oxygen saturation, and blood pressure as indicated in the plan of care
  • Communicate effectively and respectfully with a diverse client population and co-workers
  • Work under the supervision of a nurse:
  • Promptly communicating any family or client concerns or changes in status
  • Consulting with a nurse with regard to suggested changes in treatment
  • Submit required documentation in a timely manner
  • On call every other weekend
Qualifications
  • Current State CNA Certification in good standing
  • Current CPR with BLS card
  • Valid driver’s license, at least state minimum auto insurance, and an operational vehicle
  • Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing hospice operations
  • Great organization skills and an ability to work without direct supervision
  • Ability to remain calm in emergency or crisis situations, and to be close to clients who may be actively dying
  • Commitment to adhere to professional boundaries
  • Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
Physical Requirements
  • Duties require extensive standing, walking, and sitting
  • Requires lifting, positioning, pushing, and/or assistance with transferring patients
  • Requires frequent reaching, stooping, bending, kneeling, and crouching
