Come make a Difference as a Mental Health Homeless Prevention Worker in Leeds!Join our team as a Mental Health Homeless Prevention Worker, playing a pivotal role in supporting individuals within the Leeds & York Partnership Foundation Trust (LYPFT) – CMHT community. You’ll work collaboratively within an inclusive recovery-focused approach, you will work closely with Accommodation Gateway, Care Coordinators, and other housing support services to facilitate housing transfers, resettlement and secure housing options.If you have:
  • Strong communication and interpersonal skills.
  • An understanding of the Care Programme Approach (CPA).
  • The ability to work professionally with a client focus.
  • A full UK driver’s license and access to a vehicle for work.
