- Provides personal care to patients in their homes, SNFs, & RCFEs with consideration for age-related differences. Meets their Activities of Daily Living needs as assigned by RN.
- Coordinates personal care services, scheduling visits with patient/family.
- Observes and communicates changes in the patient’s condition to the primary RN and/or Team Manager and staff at facilities as needed.
- Attends discipline-specific and all-staff meetings and maintains continuing education credits for licensure.
- Recognizes needs of patients/families and responds appropriately within the scope of practice.
- Performs related tasks per plan of care which may include staffing crisis care, making deliveries, light housekeeping, and other tasks as assigned.
- Documents observations accurately according to charting standards.
- Maintains timely patient documentation using “point of care” electronic charting.
- Performs basic skills included in competencies checklist, policies/procedures and standards that are required for agency.
- Reports to primary RN or Team Manager any changes of condition, injuries of unknown origin, or patient/family/facility concerns in a timely manner.
- Adhering to the E-way standards is a requirement in performing all job duties.
- Post high school Vocational/Specialized Training.
- Minimum of 6 months experience (waived with special consideration of Team Manager.)
- Preferred experience of 1-2 years direct patient care.
- Must possess current California CHHA and CNA Certification.
- Current CA driver’s license, proof of insurance, and reliable transportation.
- Spanish speaking preferred.
- Computer skills (word processing), use of calculator, fax, and photocopier.
- Because this is a field position, must be able to drive in various conditions, including but not limited to windy/steep roads, night driving, and rural areas, and be able to enter homes with pets.
