Shift/Schedule: Full Time (40 hrs/wk), Day Shift, rotating weekendsGENERAL SUMMARYWorks under the supervision of the Nurse Manager or Asst Nurse Manager. The RN Admission Nurse evaluates and admits patients for all home care disciplines and makes regularly scheduled visits as needed. Duties include: admission; revisit; OASIS data collection; developing the plan of care; referral of ancillary services e.g. rehabilitation therapists, social worker and home health aide and completion of required documentation. Assists with QAPI programs. Performs other duties as assigned.QUALIFICATIONSEducation:High School Diploma or successful completion of an equivalent High School Exam RequiredGraduate from an accredited school of nursing requiredBSN preferredLicensure:Licensed to practice as an RN in Ohio requiredLicensed to practice as an RN in Kentucky to be obtained after hire requiredBLS within 90 days of hire requiredExperience:Prior home care experience preferredJOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONSThe following is a summary of the major job duties of this job.   • Performs admission visits. Initiates and completes the Physician's Plan of Care
in collaboration with the patient, family, physician and other health care team members. Obtains informed
consent for treatment. Informs patients both verbally and in writing of their
rights and responsibilities as patients receiving Home Care and reviews the
patient grievance procedure. Informs the patient, family or responsible party of
available insurance coverage for Home Care services and non-reimbursable
services. Provides to the patient/family information regarding the patient's
rights to formulate Advance Directives. Instructs the patient/family in
processes to follow if assistance is needed after regular office hours. Reviews
general emergency preparedness instructions with the patient/family. * Uses assessment skills and interviewing techniques to identify patient problems and patient care needs including teaching needs. Completes OASIS assessment. Collaborates with Coder and Intake Staff for correct ICD coding
  • Formulates and initiates the plan of care including Home Care discharge planning, in collaboration with the patient, family, and/or caregivers.
  • Makes home visits to render professional nursing care. Observes return demonstrations of health restoration/maintenance procedures.
  • Completes clinical records for required documentation in accordance with Home Care standards. Maintains confidentiality of patient information.
  • Serves as liaison between Home Care and physicians, SOMC hospitals, other health care team members and community agencies.
  • Communicates changes in patient status to patients, family members, or caregivers, physicians and other health care team members.
  • Participates in the 24-hour, 7 day a week on-call nursing coverage for patients/families as appropriate.
