- Perform psychosocial assessments for patients and/or families.
- Formulate, develop, and implement individualized treatment plans.
- Collaborate with Lindner Center of HOPE care coordinators, psychologists, psychiatrists and other treatment team members.
- Champion the patient experience; the sum of all interactions that influence a patient’s perception of our continuum of care and ultimately their well-being.
- Serve as the primary point of contact and clinical liaison for all treatment team members, families, patients, referrers, and external stakeholders. Interprets and integrates all information gathered and communicates treatment goals effectively to all involved.
- Collaborate with marketing to develop and maintain close relationships with key stakeholders to maximize these relationships and act as a Lindner Center of HOPE brand ambassador.
- Perform all required documentation accurately and within expected time. Documentation is concise, pertinent and meets professional and department documentation standards. Oral reporting is clear, concise, relevant, and timely.
- Liaison with community agencies; participate in area activities related to mental health.
- Maintain current licensure and participate in continuing education to maintain competency in area of practice.
- Participate in departmental and LCOH quality improvement efforts and meets peer review standards.
- Participate in community education and outreach.
- Experience with Dialectical Behavioral Therapy, Cognitive Behavioral Therapy, or Psychodynamic Therapies is ideal.
- Strong interpersonal skills: ability to collaborate and work with others to insure the best clinical outcome for patients and families.
