Our clinical approach is person-centered and tailored to address the complex interaction of an individual's substance use and psychiatric-related issues.We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. As an LPN at Partners in Recovery, you will play a crucial role in helping individuals on their path to recovery by providing essential medical and nursing support.Key Responsibilities:
Administer medication under the supervision of a Registered Nurse or Nurse Practitioner.
Monitor and assess patients' physical and mental health status.
Collaborate with the treatment team to develop and implement individualized care plans.
Provide education and support to patients and their families.
Maintain accurate medical records and documentation.
Assist in coordinating and organizing medication management services.
Participate in team meetings and contribute to the development of comprehensive treatment plans.
Ensure the safety and well-being of all patients in our care.
