- Develop and monitor implementation of Migrant Health and Outreach Program activities according to the SJFMC Health Care Plan
- Ensure the provision of the outreach services to migrant/seasonal farm workers by coordinating such services as nursing, health screenings, health education, transportation, case management and mobile van services
- Develop, implement and evaluate policies, procedures, business plans and special projects that support the provision of migrant/seasonal health services
- Perform continual assessment on policies and procedures to ensure compliance with local, state and federal regulations, laws & contracts
- Determine the qualifications and competence of department personnel; continually assess the competency of all staff and complete written performance reviews in a timely manner
- Provide opportunities for staff development by establishing and implementing educational plans for department in-services and orientation, unit specific requirements and mandatory organizational education; provide for the appropriate education for staff members
- Recommend required resources as needed to enhance outreach services
- Establish and maintain satisfactory relationships with personnel in all operating departments of the company, patients, doctors and the business/medical community at large
- Develop and implement the health education programs and special education programs such as ESL classes
- Serve as a liaison and/or interpreter between farm workers, community organizations and health center staff
- Perform group health education sessions and activities at the labor camps and health centers
- Coordinate transportation for farm workers to community organizations and health care facilities; maintain reports of vehicle maintenance; monitor GPS systems
- Visit campsites in a 5 county radius of South Jersey; identify health problems and concerns of farm workers
- Provide appropriate referrals specialists, clinics and other community resource
- Responsible for remaining current with business, health trends, and innovative industry practices.
- Perform all other tasks as assigned by the President/CEO
- Master's in health education, public health, nutrition or a related field
- Minimum of 3 years' experience in health education, including experience in a supervisory role
- Bilingual in Spanish or Creole preferred
- Excellent written and verbal communications skills
- Strong interpersonal skills and the ability to work in a team environment.
- Valid driver's license and reliable transportation
- Knowledge of local community resources is a plus
