We have 4 positions available for individuals who are committed to making a positive impact in the lives of older adults in our community.Position Type: Full-time, Permanent
Location: Hertfordshire County Council Offices (3 days office, 2 days WFH)
Note: Mixed work arrangements can be discussed.Key Responsibilities:Assessing the needs of older adults and their families, particularly in relation to continuing health care.
Developing and implementing care plans that promote the well-being and independence of older people.
Collaborating with healthcare professionals, service providers, and community resources to ensure comprehensive support for clients.
Advocating for the rights and best interests of older adults, including safeguarding vulnerable individuals.
Providing emotional support, guidance, and practical assistance to clients and their families.
Qualifications and Experience:Bachelors degree in Social Work or a related field.
Registration with Social Work England.
Experience working with older adults and/or individuals with sensory impairments preferred.
Strong assessment, communication, and interpersonal skills.
Ability to work effectively both independently and as part of a multidisciplinary team.
