This post is 12 months fixed term, with a view to continuation depending on funding.This post is in partnership with Firvale Community Hub and Foundry Primary Care Network. One day of this role will be working with communities from Firvale Community Hub with four days working as Patient Access Care Coordinator for Foundry PCN.This post will be employed by Firvale Community Hub and will adhere to Firvale Community Hub’s employment terms and conditions. The post will be jointly supervised by Firvale Community Hub and Foundry Primary Care Network.The postholder provides support to a cohort of vulnerable patients across the Primary Care Network to access hospital and outpatients’ appointments. The postholder has a good understanding of health inequality and recognises that some people experience worse health outcomes due to the wider social and economic determinants of health.The role will be predominately administrative. The post holder will receive referrals and will undertake proactive searches to identify patients at risk of not attending hospital appointments. The postholder will support people to access their hospital appointments using knowledge of processes, entitlements, services and referral pathways that are in place to support people to access appointments . The postholder will support patients and, where appropriate their families and carers, to navigate these systems.Working in the diverse and vibrant communities that is the Foundry Primary Care Network, the Care Coordinator recognises the additional barriers and challenges that prevent people from accessing important health appointments.
