- Evaluate the client’s level of functioning and determine the type of speech and or language disorder and appropriate corrective therapy
- Completes thorough and knowledgeable in-home assessment, implementation and revision of care plan to improve client’s abilities
- Observes, records, and reports any change in the client’s condition, response to the therapy and/or suggestions for changes in the plan of treatment/care to the physician
- Follows all regulations, laws and policies involved with the plan of care
- Participates as a member of the interdisciplinary team conference and confers on the client’s progress whenever needed
- Prepares clinical records (initial evaluations, plans of care, progress notes, discharge summaries) and submits them for the client clinical record in a timely manner
- Instructs other health care personnel and staff members in methods of assisting the client to improve and correct speech disabilities
- Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

- Knowledge and comfort level working with technology and enterprise wide support systems and tools
- Successful management of a high volume of work, changing priorities as needed
- Appropriate judgment and critical thinking in decisions that requiring consideration of regulatory compliance, customer service, patient welfare and company interests
- Outstanding presentation, writing and communication skills
- Promotes the agency through positive representation and communication of its services
- Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
- Creative thinker who embraces thought leadership
- Capable of making challenging decisions and willing to take responsibility for them.
- One (1) year experience as a Speech/Language Pathologist or as required by law
- Possess a certificate of clinical competence in speech/language pathology granted by the American Speech/Language/Hearing Association.
- Current CPR certification
- Valid driver’s license and automobile insurance
