Medical Director for Private Ambulance Operations: A physician appointed by an approved EMS Private Ambulance Operator and endorsed by the Los Angeles County EMS Agency Medical Director, responsible for overseeing all medication usage by EMTs and paramedics, including controlled substances. Additionally, they supervise the private provider agency's quality improvement initiatives, as outlined by the Los Angeles County EMS Agency.The duties of the Private Ambulance Operator Medical Director include:
  • Possessing board eligibility or certification by either the American Board of Emergency Medicine or the American Osteopathic Board of Emergency Medicine in Emergency Medical Services or Emergency Medicine, with evidence of significant experience and practice in EMS.
  • Actively involved in the practice, supervision, or teaching of emergency medicine and/or EMS.
  • Demonstrating knowledge of the current policies, procedures, and protocols of the Los Angeles County EMS Agency.
  • Attending an EMS system orientation provided by the EMS Agency within six (6) months of employment.
  • Participating in the annual program review(s) – BLS, ALS, SCT, or contributing to the exit summary.
PRINCIPLE: Medical Directors play a vital role in improving the quality of prehospital care by offering medical expertise in EMS and facilitating communication between the EMS Agency Medical Director, hospitals, and other Private and Public Ambulance Operator Medical Directors to ensure the delivery of safe and effective medical care.POLICY:Role and Responsibilities Of The Private Operator Medical Director:
  • Medical Direction and Supervision of Patient Care
  • Advising the private ambulance operator in planning and assessing the delivery of prehospital medical care by EMTs, paramedics, nurses, and respiratory therapists if applicable.
  • Reviewing and endorsing the medical content of all EMS training provided by the private ambulance operator. Ensuring compliance with State and local EMS Agency continuing education requirements if approved as a continuing education provider in Los Angeles County.
  • Reviewing and endorsing the medical aspects of the private ambulance operator's dispatch policies and procedures through signature or other approved mechanisms, such as electronic signature.
  • Contributing to the development of policies and procedures to enhance patient care.
  • Ensuring compliance with legal documentation requirements for patient care.
  • Providing oversight and actively participating in the private ambulance operator’s Quality Improvement program.
  • Ensuring compliance with Los Angeles County EMS Agency controlled substance policies and procedures if applicable.
  • Participating in relevant EMS committees and the local medical community as necessary.
  • Attending at least 50% of the Medical Advisory Council meetings or designating a representative for information gathering in their absence.
  • Audit and Evaluation of Patient Care
  • Assisting the private ambulance operator in establishing and implementing a continuous quality improvement program to uphold the delivery of quality medical care. Offering recommendations for training and operational adjustments based on quality improvement findings.
  • Evaluating private ambulance operator medical personnel to ensure adherence to the medical policies, procedures, and protocols of the Los Angeles County EMS Agency.
  • Conducting periodic reviews of dispatch and patient care records to identify potential issues in patient care.
  • Reviewing patient care delivery and evaluation with base and receiving hospitals as required.
  • Investigation of Medical Care Issues
  • Reviewing incidents involving unusual or adverse patient outcomes, substandard performance by EMS personnel, and complaints related to medical care delivery.
  • Evaluating medical performance and providing relevant details in writing to the Los Angeles County EMS Agency Medical Director as necessary.
  • Ensuring appropriate actions, such as training or counseling, are taken regarding patient care issues with adverse outcomes or near misses.
