Home Care Service Coordinator (SW I) (Contract) (Continuing Care - Enhanced Home and Community Care Services)

• RSW, Degree holder in Social Work or above

• At least 5 years' post degree Social Worker experience in elderly service, especially for IHCST / EHCCS is preferred

• Good command of written & spoken English and Chinese

• Dynamic, efficient, independent, responsive and resourceful

• Good interpersonal and team work skills

• Execute the strategic planning of Continuing Care (Enhanced Home And Community Care Services)

• Supervise Enhanced Home And Community Care Services to ensure excellent service quality and up-to-date service delivery

• Manage existing provision and initiate fill-the-gap projects for service users, their families and society at large

• Perform any ad-hoc duties assigned by supervisor

• Work Location : Hong Kong Island Area
