- Registered Social Worker in Hong Kong with a recognized degree in Social Work
- Minimum 7 years of working experience in rehabilitation community services or related service
- Manage the overall planning & operation of a pilot project of Palliative and End of Life Care for Person with Disabilities (賽馬會之「同行．晚晴」照顧計劃)
- Liaise with local stakeholders to develop collaboration network
- Act the role as Project Manager to lead and supervise the project team and management of volunteer service
- Act the role of case manager to provide professional case and group work intervention to the service users & carers
- Provide training to the carers & volunteers
- Service promotion and public education on Palliative and End of Life Care for PWDs
- Proactive, mature and independent; possess good computer literacy
