- Monitor HEDIS gaps via the Care Coordination Tool (CCT) for each client monthly, and work with each client on closing their gaps in care within established timeframes.
- Verify client payer is active and client is active with HCA in THL program as listed in the Care Coordination Tool (CCT) once a month.
- Supports smooth transitions between care settings by coordinating information sharing and service provision with providers and the client.
- Develop or support the development of a comprehensive discharge or transition plan with steps for follow-up.
- Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns.
- Develop care plans with the input of the client, parent/guardian, treatment team, and the Licensed/Clinical Supervisor.
- Ensure all Health Link clients on your caseload receive the number of contacts required by the Health Link program each month. Provide the required # of client contacts attempts including collateral care coordination and document efforts in Carelogic.
- Refer and coordinate linking Health Link clients to other HCA services as appropriate to the client’s service needs.
- Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
- Meet with clients weekly or bi-weekly for up to 12 months. THL services may be more or less and can exceed 12 months as deemed medically necessary by Licensed/Clinical Supervisor.
- Attend and participate bi-weekly with Treatment Team meetings and individual clinical supervision meetings with the Licensed/Clinical Supervisor.
- Attend and participate in weekly THL Team meetings.
- Outreach to THL attributed members to include program education, scheduling of assessment for program enrollment, MCO portal opt-outs, and documentation of outreach efforts.
- Serve as a liaison with other professionals and agencies as related to the client..
- Participate in the PQI process and strategic planning process.
- Employee will include sensitivity to the service population's cultural and socioeconomic characteristics in providing treatment services.
Bachelor’s degree required with preference to human service fields or other health related discipline.
