LocationRoyal Derby Hospital, Uttoxeter Road, DerbyReferenceJOB/24/01770Salary detailsGrade 28 £41,765 - £45,149 per annumJob termFull TimeAppointment typeFixed Term - Covering for secondment until 31/12/2024Hours37 hours per weekClosing Date26 May 2024We are seeking to recruit an experienced Senior Practitioner to join the Hospital Discharge Team, based at University Hospitals Derby and Burton to cover a secondment.Experience of working in a hospital setting beneficial but not essential, you will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital. This post will require you work flexibly Royal Derby Hospital and Queens Burton Hospital to maintain system flow.The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. This this change prior notice will be given in advance.You will need to be able to manage change and have the ability to maintain positive relationship with health partners and other agencies. You will need to be committed to working in a person centred way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.Employees have access to a range of benefits and discounts including:
