We have an exciting opportunity for a Primary Care Nurse- Cardiac Lead to join our Primary Care team at HMP Wakefield. HMP Wakefield is a category A men's Prison located in Wakefield, West Yorkshire.HoursThe site currently offers a 24 hour service and have shifts patterns across these hoursThe role will require working 4 out of 7 days on a rota basis. This includes evenings, weekends and possibly some night shiftsSalaryThis position is full time, you'll receive an annual salary up to £46,200 per annum - depending on experience.Qualification
  • Registered nurse - Adult with current NMC registration
  • Qualifications in cardiac rehab and the heart manual would be beneficial

Candidates are required to have a Legal right to work in the UK on a permanent basis and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.What you'll be doingAs a Primary Care Nurse- Cardiac Lead, your responsibilities will vary. You will
  • Be responsible for the monitoring all cardiac long term conditions, cardiac rehab, heart manual and the management of the emergency bags and equipment.
  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team
