EMR System: PointCare About the Role:We are seeking a dedicated and skilled RN Field Case Manager to join our growing Home Health team in Staten Island, NY. This role involves providing comprehensive patient care, coordinating services, and ensuring quality outcomes. We are open to training RNs without prior OASIS experience and offer growth opportunities into leadership roles. Key Responsibilities:Admit and reassess home health patients based on physician orders.Initiate and update individualized plans of care under doctor’s supervision.Monitor, assess, and document patient progress and report any changes.Educate patients and caregivers on disease management, medication, and care plans.Coordinate and supervise HHAs in patient care and discharge planning.Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care.Participate in ongoing training, including infusion care specialization. Qualifications:Active RN license in New York State (in good standing).Home health experience preferred, but we are willing to train nurses without OASIS experience.Strong clinical assessment, documentation, and communication skills.Proficiency with PointCare EMR (preferred but not required).Reliable transportation for field visits (mileage reimbursement provided).
