The successful candidates will play a crucial role in providing support to patients and their families, conducting assessments, and coordinating care plans within a hospital environment. This position offers a rewarding opportunity to make a positive impact on the lives of individuals in need of social care support.Key Responsibilities:Conducting social work assessments and interventions with patients and their families in hospital settings.
Collaborating with multidisciplinary teams to develop and implement holistic care plans.
Providing emotional support and advocacy for patients and families during their hospital stay.
Liaising with community services and external agencies to facilitate smooth transitions for patients upon discharge.
Maintaining accurate and up-to-date documentation of assessments and interventions.Requirements:Qualified Social Worker with relevant registration (e.g., HCPC).
Proven experience working in a hospital or healthcare setting, preferably with experience in adult social care.
Ability to travel to Calderdale Royal Hospital and Huddersfield Royal Infirmary for assessments as required.
Flexibility to be office-based three days a week, with negotiable options for two days a week.
Strong communication and interpersonal skills, with the ability to engage effectively with patients, families, and colleagues.
Familiarity with the use of CIS system preferred but not essential.Benefits:Competitive hourly rate of £35 per hour.
