- Licensed or eligible for license in Kentucky
- Basic Life Support (BLS) accredited by the American Heart Association (AHA)
- Advanced Cardiac Life Support (ACLS) accredited by the American Heart Association (AHA)
- Associate’s degree from accredited college or University
- Bachelor's or Associate's degree from accredited college or University
