Job introductionWe are seeking an Occupational Therapist to work within the Reablement service at Craven Vale Resource Centre – Brighton.Craven Vale is a care setting encompassing 12 Adult Social Care funded reablement beds and 2 Bariatric Health funded reablement beds. We provide reablement support to clients for up to 6 weeks, facilitating safe and effective discharges back home.   • To respond to referrals, prioritise need and develop caseload management skills.
  • To provide timely assessments to establish a client’s functional ability.
  • To set SMART goals in collaboration with the client.
  • To facilitate the timely discharge of clients.
  • To plan, carry out and evaluate appropriate assessments and treatment programmes to increase/maintain the safety and independence of clients.
  • To assess the home environment and promote independence and minimise risk by recommending and providing assistive devices and adaptations.
  • To refer onto appropriate community services within Adult Social Care, Health and Voluntary organisations.
  • To be involved in the day to day running of the Occupational Therapy service, as appropriate.
  • To work alongside care officers to optimise the client’s functional potential, discussing OT recommendations where necessary.
  • To attend and contribute to multidisciplinary meetings, developing good working relationships with other MDT members.
  • To communicate relevant information to others involved in co-ordinating care, both written and verbally.
  • To attend appropriate training when available, e.g. Adult Social Care reporting systems (Eclipse)
