Main Duties:
  • Conduct Care Act assessments of individuals who have been discharged from hospital.
  • Assess and evaluate the individual's physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required.
  • Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
  • In some cases attend DST (decision support tool) meetings with the ICB (Integrated care board) to determine level of nursing need.
  • Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions.
  • Advocate for individuals' rights and entitlements, ensuring that they receive appropriate and timely support services.
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organisational and legal requirements.
  • Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.
Requirements of the Social Worker:
  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
