- Performs all physical therapy procedures, planned and delegated, as prescribed by the physician in the client’s place of residence when instructed and supervised by a qualified Physical Therapist
- Administers non-complex therapeutic exercises, therapeutic massage, traction, heat, light, cold, water and electrical modalities to clients with relatively stable conditions
- Instructs, motivates and assists clients in learning and improving functional activities
- Observes client’s progress and response to treatment and reports findings to the physical therapist
- Prepares clinical notes and progress reports documenting physical therapy treatments performed and submits them for review to the supervising Physical Therapist for inclusion into the clinical record
- Reports any changes in the client’s condition to the qualified supervising Physical Therapist and Agency Management
- Changes in the plan of care may be made following authorization by the client’s physician and subsequent authorization, instruction and supervision by the Physical Therapist
- Participates in case conferences, staff meetings, and in-service programs as required
- Submits all documentation (clinical and payroll) timely and per branch policy
- Assists with quarterly record reviews if requested
- Contributes to the effectiveness of case management and coordination of services through open communication
- Follows directives of Case Manager and supports the efforts of case coordination
- Assures appropriate documentation for all client care provided
- Is punctual for client visits and reports to work as scheduled
- Maintains appropriate boundaries with client and families; avoids significant self-disclosure; interacts with clients and families on a professional level
- Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

- Outstanding presentation, writing and communication skills

- Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
- Creative thinker who embraces thought leadership
- Graduate of a physical therapy assistant program approved by the American Physical Therapy Association
- One year home health experience providing PTA services to clients or as required by state law
- Certified as a Physical Therapy Assistant (in good standing) in the state(s) of practice
- Valid driver’s license and automobile insurance
