- Advise educators on potential side-effects of different classes of medications
- Provide medical consultation to educators regarding issues such as medication management for individual students, particularly those with complex medical conditions concurrent with mental illness or addictions
- Liaise with children’s mental health agencies and primary care practitioners as required
- Provide support and/or intervention in complex issues such as refusal to attend treatment, self-harm, suicide, or violent behaviour
- Support educators to meet the complex medical and mental health needs of students who require extra supports for health and/or safety concerns of self and/or others
- Provide the support of a health professional in helping students and/or parent(s) to access services such as primary health care or a children’s mental health centre and provide education about options for treatment.
- Provide essential health related advice and support to the Mental Health leaders in developing comprehensive board plans to address a collaborative response to the mental health needs of students.
- Act as a spokesperson as required, and interpret the role of Home and Community Care Support Services to patients, health care professionals and to the public. Ensure positive public relations and effective co-ordination of services through ongoing liaison and participation on internal and external committees.
- Participate in the orientation of new staff, educators and students within district school boards.
- Assesses for and promotes a safe environment for patients, caregivers, family members, and staff. Adhere to health and safety policies/ practices developed and implemented by Home and Community Support Services Erie St. Clair.
- Participates in establishing, maintaining, and monitoring standards for the Care Coordination. This includes committee work and active participation and contribution to quality initiatives.
- Regulated health care professional holding current registration with a regulated college in Ontario; appropriate university degree or a suitable combination of education and direct experience.
- Must be fluent in French and English - both written and verbal skills
- Current registration as an RN/BScN in good standing with the College of Nurses of Ontario
- 2-3 years of relevant experience required, experience in providing direct clinical mental health and/or addictions services for children and youth preferred
- Sound knowledge of the Ontario health care system; the role of Care Coordinator as assessor and health planner, all relevant legislation and available local community resources
- Solid ability to use MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.) and internet research skills and computer navigation
- Ability to analyze information, problem-solve and make good decisions
- Accountable for own actions and decisions, making decisions within the scope of the position and referring issues/problems/events to the manager as required
- Solid documentation skills (clear, thorough, accurate and timely)
