- Provides patient and family focused care within the RN scope of practice, based on the three factor framework of the College of Nurses of Ontario
- Functions in accordance with established hospital policies and procedures, including emergency codes.
- Ensures that practice is consistent with entry to practice competencies, standards of practice, guidelines and legislation.
- Communicates effectively with patients, families and all members of the healthcare team.
- Practices collaboratively with members of the healthcare team to provide elements of care for patients/clients with complex health issues.
- Practices collaboratively with other members of the interprofessional health care team.
- Provides consultation and/or clinical support to peers as needed.
- Contributes to the assessment and documentation of each patient's health status on an ongoing basis.
- Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.
- Assists in the orientation/preceptorship of new staff members and students to the clinical units.
- Acts as a role model and champion to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care.
- Takes a leadership role in the coordination of patient care in assigned area
- Assumes charge nurse role when assigned
- Assists in the orientation/preceptorship of new staff members and students
- Effectively utilizes human and material resources
- Participates in the development, implementation and evaluation of clinical pathways
- Provides consultation and/or clinical support to peers as needed
- Other duties as assigned by Manager/delegate
- Demonstrates the ability to perform initial and ongoing health assessments, either in person or virtual assessments, that identify factors that will impact the patient's cancer experience,
- Demonstrates the ability to formulate a plan of care with the patient, family and inter-professional care team to address issues identified during assessments, implement and evaluate the outcomes of best evidence based care,
- Demonstrates the application of evidence informed knowledge, skill and judgement in the management of cancer treatment modalities including: administration, symptom management, safe handling, adverse events as a consequence of disease and or treatment
- Demonstrates understanding of and utilizes principles of monitoring of patient during blood and blood product transfusion and other supportive care treatments,
- Demonstrates ability to recognize and critically analyses situations for potential ethical and legal issues and apply ethical frameworks to support patients and families' decision making.
- Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
- BScN preferred
- Certification in chemotherapy administration from a recognized institution (de Souza Institute/ Oncology Nurses Society [ONS]) within the first 6 months of hire, demonstration of annual recertification.
- Learning Essential Approaches to Palliative Care (LEAP) certification preferred
- CON (C) in oncology preferred
- Active member of Canadian Association of Nurses in Oncology(CANO) preferred
- Minimum of 1 year recent (within the last five years) and relevant oncology/palliative nursing experience in an acute care setting preferred
- Knowledge of the philosophy and concepts of Oncology and Palliative Nursing practice required
- Minimum of one (1) year of experience with the initiation of IV therapy and central venous access devices required
- Knowledge/Skill/Judgement to access central venous access devices required, or willing to obtain within first 3 months of hire.
- Minimum of 1-3 years of experience as a primary nurse in an ambulatory clinic environment preferred
- Minimum of 1-3 years of experience with telephone triage nursing principles and practices preferred
- Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines and budget in accordance to Southlake Regional Health Centre standards.
