As Royal Freemasons continues to make a positive difference to the lives of older people across Victoria, the organisation seeks to appoint a Clinical Care Manager at Royal Freemasons Residential Aged Care – Springtime.The roleWorking within the leadership team the Clinical Care Manager (CCM) will lead, motivate & develop the clinical team ensuring a high level of person-centred clinical care is provided to residents.Utilising excellent communication & leadership skills, the CCM provides overall clinical guidance and advice to residents surrounding their clinical care, undertaking resident case conferences and assessments ensuring resident needs are met and maintained in accordance with the care plans.Key responsibilities include
  • Educating, mentoring, and supporting care staff to deliver a high level of quality care.
  • Supporting the admission of new residents including assessments, care planning, and reviews.
  • Managing complex healthcare including wound care, infection control, pain management, falls, and continence issues.
  • Leading the accurate and timely completion of reporting and documentation in line with relevant legislation, regulatory requirements, professional standards, policies, and procedures.
  • Providing excellent, person-centred resident care in relation to each individual’s needs, including undertaking resident and family consultation in regard to all clinical care.
  • Ensuring resident assessments, initial care plans/reviews and other relevant clinical documentation are completed accurately and reflect the complete individualised needs of the resident and legislative requirements.
  • Supporting the Facility Manager with the general management of the facility and assuming responsibility for the overall management of the site in the absence of the Facility Manager.
Knowledge and experience
  • Registered Nurse with current and applicable registration with the Nursing and Midwifery Board of Australia (AHPRA).
  • Post graduate qualifications and / or relevant experience in aged care will be favourably considered.
  • Demonstrated knowledge and understanding of aged care standards, accreditation and auditing processes, and AN-ACC.
  • Experience in residential care services as a Clinical Care Coordinator or equivalent with the proven ability to manage day-to-day clinical operations.
  • Experience in Incident report management, AN-ACC validations, Care Planning, assessments and documentation, specialised nursing care.
