About the RoleThe primary responsibility of this role is to provide clinical care and professional advice and support as part of the Check In Team based at Frontyard Youth Services ensuring delivery of an efficient, effective and clinically therapeutic service.The Senior OT Mental Health Care Coordinator will provide support to the Grade 2 OT and contribute to the leadership of the Check In program.Additionally the role will coordinate short to medium term specialist assessment and targeted therapeutic interventions to clients of the Check In program. Interventions provided will be aimed at supporting psychosocial recovery including fostering individual recovery, independence, and improved health outcomes for clients with complex needs. Interventions and assessments provided could include, but not be limited to, function and capacity, sensory modulation, social skills, activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs. Interventions provided will either be on either a 1:1 basis or delivered within a small group.Additionally as a care coordinator, this role will deliver holistic health assessments and facilitate client driven goal setting and care planning, maintaining an active caseload and supporting clients to develop self-management skills in relation to their needs.This position will receive clinical/professional support from the Senior Care Coordinator within the Check In Program, and through the ALERT leadership team. Operational leadership onsite will sit with Frontyard.Classifications for this position will range from Occupational Therapist Gr 3:
YB24 - YB27 ($99,606 - $113,796) per annum

HfH comprises several services that support clients that are experiencing or at risk of homelessness. HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients.Operationally the role is based at Frontyard Youth Services which is a specialist youth service located in Melbourne’s CBD. Frontyard aims to support young people aged 12 to 24 to meet their physical, emotional and social needs and to develop pathways out of homelessness. This includes but is not limited to providing 18 beds crisis accommodation service and drop in center housing multiple services working with young people across Greater Melbourne and throughout Victoria.Working at St Vincent’s
