Job Description:a) Consistently complies with the facility policies, procedures and practices.
b) Supervises and organizes the assigned section within the department.
c) Reviews medications orders and patient medication profiles for appropriateness of therapy.
d) Monitors for drug interactions, duplication of therapy, allergies, adverse reactions, appropriate formulary alternatives and identifies medications.
e) Processes medications orders (example: medication order entry and verification.)
f) Maintains proper documentation on patient profiles or prescriptions.
g) Identifies and capable of using resources to ensure safe medication practices.
h) Recommends changes in medication therapy based on appropriateness and medication management initiatives (example: policies)
i) Responds to incidences, adverse drug reactions, medication errors, and reports appropriately.
j) Checks all prescriptions/physicians orders for accuracy before dispensing.
k) Checks pre-packing, ward stocks, pharmacy trays (and any other pharmacy kits) prepared by pharmacy support staff.
l) Dispenses medications to patients/nurses
m) Counsels patients/nurses with clear and accurate instructions
n) Calculates and confirms pharmaceutical dosage calculations
o) Removes and replaces expired medication
p) Prints computer fill lists, reports, labels as needed to prepare medications for dispensing.
q) Performs tasks related to health insurance and billing (as required).
r) Responds to physician, health care workers and patient drug related questions appropriately.
s) Participation in the organization's quality improvement and safety activities.DECISION MAKING AUTHORITY:
  • Work is carried out within general policies, principles and goals, and is subject to direction only for guidance on policy issues.
ACCOUNTIBILITY:
  • Accountable for day to day related assignments
OCCUPATIONAL SAFETY AND HEALTH MANAGEMENT:
  • Attending Occupational Safety and Health (OSH) programs as a mandatory requirement.
  • Being compliant with all OSH policies & procedures and legal requirements regarding Occupational Safety and Health Management System.
  • Conducting his/her task in a healthy and safe manner at all times.
  • Reporting any untoward events that will impact the occupational, safety & health of patients, staff, contractors, facilities, operations, etc.
  • Comply with the established OHSMS procedures of which they have been informed, instructed and trained.
  • Actively participate along with line management towards implementation of OHSMS requirements.
  • Report to the respective manager any potentially hazard, unsafe act or condition, which may cause EHS impacts.
  • Report all EHS hazard, near miss and incidents to the respective line manager.
INFECTION CONTROL:
  • All employees are personally accountable for their actions and are responsible for ensuring that they comply with infection prevention and control policies.
  • Health care workers are responsible for identifying infectious conditions that require specific controls to protect themselves and their patients. In addition, they are responsible for notifying the infection prevention.

Qualifications:Bachelor's Degree in Pharmacy
At least 2years experienced in Medical Centre or similar organization . Must have DOH license.
