You will to work in partnership with healthcare providers providing clinical reviews and medicines management in line with current Department of Health guidance across a wide range of therapeutic areas.Full time Monday- Friday (part time 4 days per week may also be an option)TerritoriesParts of Southwest EnglandSwindon, Wiltshire, Dorset, Somerset, Devon & CornwallThe RoleOur client provides a nationwide team of field based Clinical Pharmacists working with GP Practices and ICSs to optimise patient outcomes. They deliver innovative services and solutions that use clinical expertise and analytical data to maximize therapy outcomes.You will be primarily field based but with some remote working from home covering ICSs and GP Practices in a geographical area. Working across a variety of therapy areas including Diabetes, CHD, COPD, Asthma, Stroke Prevention and Osteoporosis (and in partnership with GP Practices) you will conduct clinical audits and run bespoke patient review services providing telephone support or Pharmacist led clinics. These audits not only provide a highly regarded service to GPs and Practice Nurses they also improve the management of patients with chronic and complex conditions.Commercially focused you will be responsible for ensuring your GP Projects are completed in a timely fashion and all objectives are met. You will plan your customer appointments and clinics to run patient reviews efficiently to generate maximum results.Committed to career development our client is expanding and provides long term opportunities in Senior Clinical Pharmacist, Management, Service Development and Analytic roles.   • UK Registered Pharmacist (previous GP Practice Pharmacist experience not essential)
